The Board has updated their Self-Assessment Forms webpage.
They’ve removed some draft forms, and added some new “Current Adopted Self-Assessment Forms.”

The hyperlink name of the form is misleading.
When you open up the form, here’s the official name of the form, “Wholesaler/Third-Party Logistics Provider Self-Assessment.”
